Our services include personal care and companionship, health monitoring, skilled nursing, chronic disease management, wound care, IV therapy, medication management, catheter and ostomy care, and several other types of support for individuals aged 18 and older.
Examples include individuals with limited vision or hearing, arthritis, COPD, diabetes, congestive heart failure, and those recovering from severe medical conditions like cancer or heart disease. We also assist clients with disabilities and those returning home from the hospital after recent surgeries or changes in their health. Many of our clients face multiple conditions that make daily activities (ADLs) difficult.
Where do you provide services?
Advocate Health Care Services provides personal care, private duty, and skilled nursing services wherever you are. From nursing homes to assisted living facilities, our compassionate team offers convenient care in the comfort of your home.
Who qualifies for home healthcare services?
To be eligible for our services, an individual must be at least 18 years old and classified as homebound, which means they have difficulty leaving their home due to a medical condition and require skilled nursing or therapy services on an intermittent basis, as verified by a doctor during a face-to-face visit.
How are home healthcare services paid for?
Home healthcare services can be generally paid for through various sources, such as personal funds, private or long-term care insurance, government programs, and community organizations. The cost is based on the type and duration of care required. We are currently in the process of credentialing with insurance companies. Until then, all our services must be paid out of pocket.
Are your services covered by Medicare or insurance?
Our home healthcare services are not currently covered by Medicare or Medicaid, but we are in the process of obtaining approval. Until then, all our services must be paid out of pocket.
How are caregivers selected?
All employees undergo thorough screening during the hiring process. They are interviewed and subjected to a complete reference check, criminal background screening, and drug testing. Anyone working for Advocate Health Care Services, from home health aides to registered nurses, is bonded and insured. We strive to match caregivers with you based on the skills needed and personality traits that help build a strong relationship.
How quickly can services be arranged?
Services are arranged based on need, availability, and when the doctor submits the order. Each case is unique, but we will do our best to coordinate services in a timely manner. Please call us at 763-317-0563 to discuss your specific needs.
Are minimum hours or days required to obtain services?
Our personal care services require a minimum of two hours, while our private duty services require a minimum of four hours. However, no minimum number of days is required to receive our services. Advocate Health Care Services is flexible and recognizes that you may need to increase or decrease services as circumstances change.
